Physicians without a U.S. medical degree must undergo a specialized approval process, regardless of whether they have just completed their medical studies or have been working as a specialist or professor for several years.





Step 1: Demonstration of Equivalence of Non-U.S. Medical Degree

  • The first step involves proving the equivalence of the non-U.S. medical degree, a process consisting of two stages and spanning several years.

  • For the recognition of the German medical diploma, various university documents need to be submitted, and three American licensure exams (USMLE Step I, II CK, II CS) must be passed. The Educational Commission for Foreign Medical Graduates (ECFMG) is responsible for these steps.

  • The USMLE exams cover knowledge areas such as Anatomy, Psychology, Microbiology, Pathology, Pharmacology, Statistics, and Physiology. The exams are computer-based and have varying costs.

Step 2: Interviews & Selection of Residency Program

  • Upon successfully completing all exams, the ECFMG issues the equivalency diploma and grants access to the Electronic Residency Application Service (ERAS).

  • The application process for residency programs takes place in August, with interviews occurring between October and February. A priority list is submitted in February, and applicant-program assignments are announced in mid-March.
